Dr. Amanda Piche
Dr. Amanda Piche believes dental care should be both precise and personal. At FORMA, she blends advanced surgical expertise with a warm, patient-centered approach to help people reclaim their health, function, and confidence through dental implants and periodontal treatment.
A top graduate of the University of Iowa College of Dentistry, Dr. Piche was inducted into the prestigious Omicron Kappa Upsilon honor society and awarded the American Academy of Periodontology Dental Student Award.
Dr. Dominik Dubravec
Dr. Dubravec grew up in Herscher, IL along with his five brothers and one sister. He attained his bachelor's degree in biology from the University of Illinois at Urbana-Champaign. He then received his Doctor of Dental Surgery degree from the Loyola School of Dentistry.
Furthermore, he attended the Harvard School of Dental Medicine and was awarded a full scholarship from the National Institute of Health.

Will insurance cover the cost?
We do not accept insurance as a form of payment, but as a patient convenience we file to insurance companies. Any insurance coverage will be reimbursed to the patient directly. Additionally, most insurance companies and plans will not fully cover implant and periodontal surgical treatment.
What will it cost?
Each plan is individualized per patient depending on treatment options and needs.
Do I need x-rays?
In order to properly diagnose and create an individualized treatment plan, x-rays will likely be needed and can be taken in house. Each patient and the x-rays required will differ.
